InnVision
InnVision the Way Home is the lead agency managing the Opportunity Center services center. InnVision is a San Jose-based organization that manages over twenty sites, and has extensive experience delivering homeless services.


Opportunity Health Partners
Opportunity Health Partners (OHP) is a nonprofit corporation that provides a comprehensive range of health care services to the residents and unhoused clients of the Opportunity Center.  OHP operates a licensed community clinic that offers primary care, mental health care, case management, and other supportive services to both adults and children.  OHP has a particular focus on providing needed services and support for those with disabilities.  OHP was founded as a cooperative venture among the Community Working Group, InnVision, Stanford University School of Medicine, the Palo Alto Medical Foundation, and the Palo Alto Veterans Affairs Medical Center.

Ravenswood Family Health Center
Ravenswood Family Health Center is a non-profit community health center that provides primary medical care and prevention services for all ages, including the uninsured and new immigrants, regardless of ability to pay. The Center’s patients live in East Palo Alto, the Belle Haven community in Menlo Park, and the North Fair Oaks section of Redwood City. The Center has a staff of 85 full and part-time employees and contractors, including 24 medical professionals.
